- Driver’s License or Valid ID
- Insurance Card
- Referral letter (if required by your insurance)
- Reports, X-Rays, MRIs, CT Scans you may already have
- List of Current medications

How to Communicate With Your INOV8 Orthopedics Provider & Healthcare Team
We use the KLARA desktop and mobile app for daily patient communication. KLARA is secure, HIPAA-compliant, and streamlines all patient workflows in one place. Direct communication with the medical practices, labs, and pharmacies you use are all possible without having to make a phone call and be placed on hold. You can use KLARA to communicate with your provider through secure messaging and video conferencing.

Request for Medical Records
INOV8 Orthopedics follows the current rules of the Texas State Board of Medical Examiners (Texas Medical Board) related to the release of medical records and corresponding charges to patients and other parties. Medical records are $25.00 (minimum). To access your medical records without a fee, use our Patient Portal.
To submit a Medical Records Request or X-ray Request: Complete the Authorization for Use/Disclosure of Protected Health Information Form. You may fax or mail the form to the fax number or address at the bottom of the form.

Please include payment of fees (shown on form) with request. Payment may be mailed with request. For other payment options, please call us. Please allow up to 15 days once we receive your request and applicable fee to process your medical records and/or X-rays.
